Community Nursing Intervention

The community nursing intervention is improving the health condition of long term ill older people are big challenges now a days health care systems. This community nursing intervention is developed by Health Quality Partners .It is one of 15 different models of care coordination in randomized controlled trials in the MCCD. Assessment of the HQP program initiated in 2002. The study stated in this is planned to estimate the existence effect of the HQP program against common care till five years .The HQP model performed to lessen mortality in long term ill older patients .Restrictions of training are few less income and not white persons are registered and execution was in solitary topographical area of the US. Further study to approve these conclusions and regulate the generalizability is acceptable.
