Streamlining admission to discharge process for adult patients receiving chemotherapy
Abstract
Patients who are expected to get admitted for their chemotherapy are instructed to get their blood workup done a day prior to their admission. The clinical nurse coordinator is responsible to bridge the communication between the attending physician and patients’ family for developing the care plan from admission until discharge. Moreover; in collaboration with the IT department, online admission process application has been improvised to track patients who are due for the chemotherapy calls are generated to these patients for confirmation of their admission and plans with other instructions are given, so that necessary measures are taken even before the admission. This helps in early administration of this drug in the daytime where patient’s monitoring is done in more vigilant manner.
