The aging population and the technical success of repeat coronary artery bypass grafting (CABG) and valve repair has led to an
increase in the number of patients with multiple sternotomy, or ?redo sternotomy?. Imaging of these patients is common, as
the awareness of the location and patency of bypass grafts is important to avoid complications including damage to the ascending
aorta, right ventricle, or grafts that are adherent to the posterior table of the sternum. However, as experience grows, there
is a growing awareness that redo sternotomy carries significant risk of bleeding from mediastinal adhesions. Reformatted CT
images are currently utilized for preoperative evaluation of patients undergoing redo sternotomy. However, using standard static
imaging, evidence of adhesions is indirect and direct visualization in usually limited. Cine CT or dynamic CT, using thin and
thick section MIP images are exquisitely sensitive in identifying these adhesions and have the potential to facilitate preoperative
planning, decrease intraoperative time and improve mortality and morbidity.
Biography
Nagina Malguria M.D. completed her medical schooling at Seth GS Medical College, India and did her radiology residency at University Hospitals of
Cleveland and completed her fellowship at Yale University, New Haven. She has extensive experience and breakthrough publications in imaging of
the heart, in particular evaluation of the heart for surgical planning.
