Abstract
Purpose: The impact of obesity and diabetes in health care is concerning. The development of co-morbid conditions present challenges to the primary care provider. Novel approaches need to be considered to address this problem. The purpose of this project is to implement and evaluate a technology based self-monitoring program for weight loss in obese adults and those with Type 2 Diabetes. Methods: With the use of smart phone technology, Lose It Application, and portion control plates, the program will assist patients with BMIs over 35 to limit caloric intake and assist in the weight reduction process. In the process of weight loss, the improvement of glycemic control will be evaluated. The monitoring of weight, BMI, HbAIC at 1 month and 3 months, will be the measurement to determine outcomes. The population and setting involves a private primary care practice in an urban area. Project Goals: The immediate goal of this change project is to have participants increase eating awareness for successful weight management and utilize technology to assist in making a personal lifestyle adjustment in behavior that leads to weight loss. Our secondary goal is to create an effective change in the education strategies used by health care providers to achieve successful weight loss results over time with sustained results.
